What is a “Reportable” Incident?
According to the NDIS incident management rules, a reportable incident is the occurrence of an unwanted event that harms the overall well-being and safety of NDIS participants.
This includes serious incidents, according to the rules, such as:
- Serious harm
- Abuse or neglect of a disabled person
- Use of restrictive practices in specific circumstances

From the time an incident occurs to the time an NDIS incident report is filed, it is critical to have effective processes in place to ensure that these unfortunate events are handled in a timely and considerate manner.
How to effectively Report an Incident?
Following the occurrence of a reportable incident, NDIS providers must complete an Immediate Notification Form within 24 hours.
To ensure that the form is correctly filled, NDIS business employees must be well-trained in recording key information about reportable incidents, such as:
- What the employee saw and/or heard
- Any witnesses’ information
- The specifics of what occurred
- Avoiding the inclusion of any personal opinions, judgments, or interpretations
- Workers should relay this critical information to their superiors.
It is then the responsibility of the designated employee to report the incident.
Completing the NDIS Incident form
Each incident must be reported to the NDIA, and the following information must be provided:
- The NDIS Provider’s Name
- Who sends the notification?
- Individuals Involved
- A description of the incident that needs to be reported
- A description of the harm done to the disabled person
- The actions that were taken as a result of the incident
